Medicare and/or Medicaid participating nursing homes in Georgia may enroll:

• Employees (minimum of 2 including a CNA is highly recommended)
• 1 Resident/Elder
• 1 Family Member

When choosing individuals for project teams, choose employees who have NOT yet completed Certified Eden Associate Training or Certified Eden at Home Associate Training.

DEADLINE for submission of scholarship applications is Monday October 8, 2018.

This education will convey the WHAT, WHY, and HOW of person-centered/person-directed care, which is a key focus throughout today’s best practices and the new standard of care that is required to initiate and maintain effective person-centered/person-directed care and culture change over time. This is the heart and soul of person-centered care planning.
